How to make launch icon's for app's

I am trying to make a new launch icon, but when ever I try, I get this error:
BUILD: ERROR: Invalid palette provided for bitmap
This even if I make it in B&W with only two colors in the palette. Is there a special format to use? The default launcher_icon.png is working. I have tried to edit this in Gimp and PhotoShop with no luck.

What does your resource definition look like?

I've seen this error before, and it was because the palette definition in the resource file was bogus/broken.

Thank you. My mistake was that I had defined two bitmaps in the resource file. If I only has one it works.
