I am sorry to hear you are having this issue. There are a few initial steps I would recommend trying.
I think you also need log in your Garmin Account on the website (not Garmin Connect) and change your country to US.
I think on previous watch generations you had to restart the watch and it would go through

Pair the watch with the phone and, and try adding the Garmin Pay card.
Done all that and now it's saying please ensure Bluetooth is turned on on the phone and watch which it obviously is or it wouldn't be connected,is it just crap in general or does it actually work sometimes?
Have you used it before? i had similar problems with revolut that neither revolut, which was listed as a partner, nor garmin could explain about 3y ago, in the end it was down to choosing a Mastercard badged card rather than visa, it registered without issue
Can't get as far as adding a card.
I dont follow, is your watch registered to garmin connect, Bluetooth paired and Bluetooth connected?
Garmin pay works fine for me.
I take it from your post above the add screen either times out or you get an error message.
Just a thought, is there a security setting on your that could prevent you from sharing a CC with an App or had your Service Provider limited that.
Not sure what country you are from but, could there be a government regulation that limits this as well. I seem to remember in the past that Garmin Pay needs to be approved by each country.
I reset my phone recently and hadn't realised the card and watch weren't now registered, just reconnected and added watch and card again no issues, are phone permissions correctly set?
I don't get as far as being able to add a card,it keeps telling to restart the watch or try again when I click on Garmin pay on the connect app.
That sounds like a bug, maybe with connected apps or similar, if the app otherwise works ok, I'd raise a ticket with garmin
