I just got my first garmin (fenix 7s pro)
I tried to go to the finish setup screen in the connect app but I don’t see it in there.
I believe my software is all up to date.
what am I missing to enable the ecg feature?

I am sorry to hear you are having this issue. If you are located in the US, and are not running BETA software, I would recommend trying the following steps to correct this.

Apologies, thats just a typo. It is a 7s PRO sapphire solar.
Garmin support got me up and running. Something about the updates that took place right out of the box didn't sync to their network i guess…
It won't work in Europe, only in the US for the 7 Pro. Unfortunately Garmin wants to prioritize the Fenix 8 so they won't launch it in Europe.
Where do you live? ECG is only available in countries/areas where it has been approved by the applicable government health agency. Fairly limited at the moment https://support.garmin.com/en-CA/marine/faq/QssFZHtIOfAYNSSmzXNwY9/ although some have found work-arounds I believe

Which firmware on your watch? Menu > System > About > Software version.
Have you installed a beta software version?
Important note about ECG: Moving forward, the Beta software available for the Fenix 7 series will have the ECG feature disabled. If you would prefer to keep the ECG feature accessible on your watch, we recommend not participating in the Public Beta program. If you do install Beta software but then wish to access the ECG feature again, you can do so by backdating your watch to currently Live software (details on how to backdate software are below.)
If you choose to backdate software, after doing so you will need to set up the ECG feature again. If you'd like more information on how to set up ECG, you can find more info on this page, see the section for "ECG App Setup": https://www.garmin.com/en-US/garmin-technology/health-science/ecg/use/
The ECG feature will be made available again on versions of beta software that we anticipate becoming Live updates. In other words, you will see the ECG feature become enabled again in Beta software toward the end of our quarterly beta cycles.

I am having the same problem - brand new Fenix 7s Solar Sapphire running software v18.14. No ECG set up that I can find in the Garmin connect app?

Your watch does not have the required hardware. Only the Pro models have the necessary sensor.
ECG is only available on the Pro models, and only in the regions described in the support article.

My watch and app both said i was on v18.14, but on her end she said it looked like it was still running v12.75. Solution was to basically remove the watch entirely from my connect account and phone, then set it up new again with the new software already installed. A pain, but it worked.
