945 Lactate threshold test

This is embarrassing, but I can't seem to figure out how to get the lactate threshold test to start on my new FR 945.  I ran the test frequently on my previous Garmin watches.

Manual directions say:

  1. From the watch face, select START.
  2. Select an outdoor running activity.

    GPS is required to complete the test.

  3. Hold .
  4. Select Training > Lactate Threshold.
  5. Start the timer, and follow the on-screen instructions.

I start an outdoor run activity, use the menu button to select Lactate Threshold test, go through a number of information screens, then have a full screen message saying something like "warm up 5-10 minutes until lap button is pressed".  I'll run a few minutes and push the lap button (bottom right) and the test never starts.  In fact, when I press lap, I see that my activity never started.

What am I missing?
