Rather than check with a gadget feel your pulse against a stopwatch, then compare.
I have done this several times, sometimes it showed three times higher values then my palpation, other times while indoor cycling it showed values around heart rate rest while pedaling at 40 kph.
No need, I know it's wrong as I am in the middle of workout and my HR definitely is above 100.
Same observation. In the middle of workout HR drops to less than 70 which I know for sure is wrong

This is a common issue with all wrist-based HR monitors. Google will confirm this.
It's important that the watch (and thus the sensor) stay fixed during the workout. That means making sure it's strapped tightly. It doesn't have to hurt, just tight enough not to move.
Also these sensors work better a couple of centimeters above the wrist joint. So slide the watch up a bit before tightening.
I am sorry your Instinct OHR is not reading you well. This Support article may help: The Heart Rate Sensor on My Garmin Watch Is Not Accurate. Sometimes your other arm will be read much better too.
