According to the menu, before I save the Activities by pressing the GPS button, I should have a "calibration and save" choice. However, in my recently purchased Instinct I did not see that option. Do I need to update or someting?

Treadmill calibration appears automatically only after the first treadmill run longer than 1.5km. Later, you can open it manually:

To calibrate the treadmill distance the first time, select Save.
The device prompts you to complete the treadmill calibration.
To manually calibrate the treadmill distance after the first-time calibration, select Calibrate & Save > Yes.

The very first time when I save Treadmill Activity, Instinct did NOT prompt me for calibration (and I am over 1mil or 1.5km)
The later run, before of after SAVE, Instinct never provide such option: Calibrate & Save.
So I wonder if anyone experience similar thing, or if I need up updated my Instinct software/firmware,etc.

Well, as for myself, it worked all right, but it is already couple of version releases back, so I do not exclude a bug was introduced later.

Were you over the 1 mile on the treadmill or on the screen of Instinct? (Instinct needs to be over 1mi/1.5km, for the calibration to be offered)
That is a very good point. Even my treadmill registered >1mile,
but Instinct only has 0.01mile. So I have to walk 100miles on my treadmill. Wow! I am no sure that I can do that.
(BTW, sorry for that emoji that I cannot remove)

How many steps? Did you hold the handle bars while running on the treadmill? That would skew the step (and distance) counting, of course.
You can also try setting up the Custom Stride Length in Device Settings » Activity Tracking (though I am not 100% sure whether it is the same cofficient that is used for the treadmill too)
Scroll down and it should say "calibrate & save" then ask if you're ready to save. Select yes & then it goes to calibrate screen. If that does not show for you, adjust on either the connect app or connect web site.
Don't forget that 1 mile on a treadmill today may not be 1 mile exactly on the same treadmill tomorrow. They are not as accurate as people expect them to be.
They most accurate method would be to get a foot pod & pair with the watch.
i am having the same exact issue, after a mile, my tracker only registers about .02, which would be a very lengthy process to calibrate (i don’t even see the option otherwise)

Until recently, my treadmill distance on my Garmin matched the distance on my treadmill. I have software version 12.20. Today is the first day that I've used the treadmill function since Christmas and the instinct is registering about 1/4 of the distance as my treadmill. Additionally the watch couldn't calculate a pace for much of my run which suggests it isn't registering the steps. I'm not holding the rails. After an issue with the GPS being WAY off during a hike over the weekend, I'm becoming more and more disappointed with this watch.

What was the number of steps during that run? If you do not remember, look at the graph of daily Steps progress in Garmin Connect Web, zoom at the section of the run (select it with the mouse), then place the mouse cursor to the nearest point before the run, note the number of steps shown in the mouse-over tooltip, repeat for the nearest point after the run, and substract the values.
Also please post a screenshot of the graph - it should reveal whether the watch indeed did not register the steps during the run. And tell us what distance the treadmill reported. Did you try re-calibrating the Treadmill Activity?
