Downloaded App Icon turns to "IQ"

I find that sometime the the icon for Apps I have downloaded turn to simply IQ. I have tried removing the app, soft restarting and re-installing without any luck.
Does anyone have any ideas how I can get the icon to show properly again?

Maybe you did not allow required access on the phone or watch just lost connection with phone.

I should add that the app works perfectly each time I have tired installing it, the problem is just with the icon disappearing after a while
