Meditations show up as meditation, but then are categorized under breathwork, not a big deal I am just not clear on what would then fall under meditation? I'm using an IQ app to record meditations. Could that be a factor?

Connect IQ apps are often used when the watch doesn't support an activity type. That also means that those apps will not report as the correct activity. Check which activity type your meditations are logged as and change to the correct one. The title can say anything no matter which type it is logged as.
This is how it should look like:
The type next to my name and the icon is the meditation icon. Title says Meditation, but it doesn't really matter what it says there.
