It seems to me that the left-most columns have simply scrolled off the display. You should be able to scroll left to see them. If you hover over the column headings on the report, you can see which fields are selected to display.
You can change what columns are shown by clicking on the triangle icon that appears in any of the column headers as you hover the mouse pointer over it.
