Why do you need to have connect app open on iPhone to connect via Bluetooth
to garmin vivoactive 5.

why won’t it run in background, instead of having the app open

It could work on background, just app cannot be killed.
Some data will not be transferred and functions will not work if you kill it.

iOS forces 3rd party apps to remain open or in the foreground! Not the same for Apple Watch, of course! :)
