I see countless options on the Manual Activity screen. What option exactly do you miss?
Missing all options except for name date time and notes. No ability to select an activity like running, walking, lifting, etc
I get a network connection error too if I try to save just those 4 basic fields.
Can you post a screenshot of the manual activity screen? How do you access it exactly? Through the +activity button on the blue bar on the My Day screen, or somewhere else?

Well, I am afraid you'll nee to fix the network error first. Did you try using WiFi instead of mobile data or vice versa?
Yup tried that too. Even before the network error, the options are not there like previously.
Was able to resolve the issue by deleting the app and then reinstalling. Works now.
