The watch depends on arm movement in order to log steps. So if you're holding something like a buggy, the built-in accelerometer will not detect steps. At the very least it will be inaccurate. I know some people will place it in a hip pocket or even strap it to their ankle to get better results.
Running with a buggy/stroller/bike trailer is stabilizing the wrist to much.
To get more steps, try this:
If possible, push only with the other hand. If not, loosen your grip at least on the watch hand. I run a lot with the bike trailer from qeerido and it is easy possible to push it only with one hand. Its a 3 wheel configuration with a fixed front wheel.
