need csv record layout to key in a printed out list of waypoints to input

I have a printed out list of waypoints
I know I can use a spreadsheet to key in the list then use a convert utility to convert the saved csv file into gpx format.

question I have is how to layout the fields in the spreadsheet. IE lat,lon, waypoint name etc

appreciate the help

Depends on the utility you're going to use...

JaVaWa RTWtool has a CSV import wizard where you can assign fields to GPX elements, so it doesn't really matter which layout you use in the CSV file.
