Treadmill recording - distance and heart rate not correct (heart rate works if switch to "cardio" recording instead)

I have a 3 year old Forerunner 45. 

I have 2 problems with the "Treadmill" setting. 1. During these activities my heart rate is not recorded correctly (e.g., it says ~100 no matter how hard I'm going). If I switch to logging a "Cardio" activity while I'm on the treadmill instead then the heartrate will measure correctly (say, ~160 instead).  2. Really incorrect calibration (after the first-run calibration has been done). Usually logs about half the distance I've actually covered, if not less.  

I reset the watch back to factory settings a couple of weeks ago and re-did the original treadmill calibration to see if this helped. Both the distance and heart-rates logged correctly for a few treadmill runs after this - but now the problem has started happening again.
